While many practices offer concierge primary care or direct primary care memberships, our Elite level includes exclusive features such as:

• Direct phone access to your provider, day or night with the ability to test for specific issues rather than require an appointment.

• One complimentary office or mobile visit per month (within 20 miles of Performance Health AZ)

• One complimentary virtual visit per month

• Personal advocacy, including up to 20 hours per year where your provider can accompany you to specialist consultations, hospital visits, or in-home skilled care This level is specifically tailored for busy professionals, athletes, and families who expect executive health solutions with complete accessibility.

• Custom-built plans that can include all of your healthcare needs - from supplements and hand delivery of prescriptions to weight loss medications and hormones.

We cap enrollment at a very limited number of Elite Concierge patients to ensure that each member receives priority access and deeply personalized attention. Once the cap is reached, new members will be placed on a waiting list. Our current cap is 10 households.
